The real cost drivers
Square footage, but not linearly. Bigger costs more, but mobilization (equipment, crew, forming) is a fixed cost spread across the job. Small driveways cost more per square foot than large ones.

Removal. Tearing out and hauling an existing cracked driveway is real work that shows up as a real line item.
